The purpose of the study was to assess the effectiveness of BIC/FTC/TAF in treatment-experienced people with HIV using available real-world cohort studies. Method: Systematic review and meta-analysis of articles/communications identified via Boolean search in Medline/Pubmed/Embase and conference abstracts reporting real-world use of BIC/FTC/TAF, published/presented until August 31, 2022. The primary endpoint was the proportion of treatment-experienced people with HIV with viral load (VL) <50 copies/mL at 48 weeks whilst on treatment. The Freeman-Tukey double arcsine transformation determined the random pooled proportion. The I statistic describes the percentage of total variation due to inter-study heterogeneity. Publication bias was assessed with the Doi plot and LFK index. The Newcastle-Ottawa Quality Assessment Scale for cohort studies adapted to one-arm cohorts was used to assess the risk of bias. Results: Of the 24 identified articles and abstracts to conferences, we ultimately included five articles (Figure 1) including 1,812 participants. Regarding full-text articles, the proportion of VL suppression in all included studies was 93% and above, and the pooled estimate of VL suppression was 95% (CI, 93%-97%) (Figure 2). However, there was moderate heterogeneity (71%) and significant asymmetry (LFK index=2.7), which disappeared after we removed Lazzaro A (I =0% and LFK=0.52) from the analysis and obtained a similar pooled proportion, 94% (CI, 93%-95%) with the remaining four publications. Total discontinuations at 48 weeks were 3% (CI, 1%-6%), 2% (CI, 1%-4%) due to toxicity. In persons carrying M184V substitution (three articles, 108 participants), supression rate was 92% (CI, 86%-97%). Figure 1: PRISMA flow diagram of studies selection procedure. Figure 2.
