EACS2023: 29 Effects of albuvirtide intensification on CD4 T cell reconstitution in acquiring HIV clients with incomplete immune reconstitution P. Ma , L. Gao , D. Zhang , R. Huang , A. Yu , H. Xia Tianjin Second People's Hospital, Department of Infectious Diseases, Tianjin, China General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: Individuals with incomplete immune recovery during suppressive antiretroviral therapy (ART) have been still represented a clinical issue. Albuvirtide (ABT) is a long acting HIV fusion inhibitor with less DDIs and rapid onset rate. This study aimed to evaluate the effect of intensive treatment with Albuvirtide on immune function in HIV-acquired clients with incomplete immune reconstitution. Method: In this prospective, open-label, controlled study, participants with incomplete immune reconstitution (continuous ART over 5 years, CD4+ T lymphocyte absolute counts <500 cells/uL or ART for 2-5 years, CD4+ counts <200 cells/uL with undetectable viral) were received ABT as intensive treatment or maintained on the original ART regimen. The participants in the intensive group received 320mg ABT daily for the first 3 days and then weekly by intravenous infusion up to week 12. The change of immune response and safety at week-12 were evaluated. Results: Twenty participants were included in each group. There were no significant differences in age, gender, time from the first ART, and current ART regimen between the two groups (P>0.05). All participants had an undetectable viral load during the treatment period. Changes in CD4+ counts significantly increased in the intensive group compared to the control group (46.83±71.39 vs 5.97±52.46 cells/uL, P=0.046 ). After the 12-week treatment period, 35% participants in intensive group had an increase at least 100 cell/uL or a 30% change inthe CD4+ counts from baseline at 12-week whereas only 10% participants in the control group achieved immunological recovery. No adverse events were considered to be related to ABT. Conclusions: Data suggested Intensive treatment with Albuvirtide was safe and effective for HIV-acquired subjects with incomplete immune reconstitution. In view of restoration of CD4+ counts in 12-week of treatment, Albuvirtide may improve the immune function recovery. General conditions 1.
