This study aimed to describe frailty prevalence in Cypriot PWH and investigate its impact on the 4 pillar of the care cascade beyond viral suppression: good health-related QoL. Method: Demographic and clinical data were collected cross-sectionally from a sample of 150 PWH in the only HIV clinic in Cyprus (~1200 outpatients actively in follow-up) between November 2021 and June 2022. Frailty was assessed using Fried's frailty phenotype, classifying patients as robust, pre-frail, or frail. QoL was assessed using EQ-5D-5L self-reported questionnaire, with good QoL being considered the report of no and/or slight problems in all dimensions. Results: Median age was 41 years (IQR=15) and 96% were male. All participants were on ART and 93.9% had a viral load <50copies/mL. Approximately half of virally suppressed participants (48.1%) reported good QoL. Weak grip strength was the most prevalent frailty criterion (57%), followed by self-reported exhaustion (24.8%), unintentional weight loss (16.8%), low physical activity (10.1%), and slow walking speed (0.7%). Based on the above, 28.9% of participants were robust, 63.8% prefrail, and 7.4% frail. For virally suppressed participants, frailty phenotype had a significant impact on health-related QoL (p=0.012) ( Figure 1). Anxiety/depression was the most affected QoL dimension, with only 56% having no or slight problems ( Figure 2). Conclusions: Although most PWH on ART in Cyprus are virologically suppressed, only half of them report a good QoL. Frailty greatly impacts QoL, and a worrisome observation is that only less than one-third of participants are robust per Fried’s criteria. Therefore, dealing with frailty is paramount in reaching the 4 90 target. Anxiety/depression is the QoL dimension that needs the greatest clinical attention. General conditions 1.
