Barchi Guys and St Thomas' NHS Foundation Trust, London, United Kingdom General data Abstract category: Access and delivery of care Abstract body Purpose: Pharmacists gained the ability to independently prescribe in 2006, becoming the second non-medical prescribing profession after nurses. Now pharmacists undertake specialist clinics in many specialties and are able to aid the MultiDisciplinary team (MDT). Their extensive knowledge on medications and position as the first point of contact means they’re in prime position to aid people with their medication-related concerns, counsel around adherence, make appropriate interventions and prescribe medicines through brief consultations on an emergency basis. This review demonstrates the impact of pharmacist prescribers in a large HIV clinic and how they ensure continuity of care by avoiding interruptions in ARV treatment. Method: Data was collected retrospectively using an emergency prescription database already established by the pharmacy team. The following information was collected from this database the reason for the prescription, which drugs were prescribed, the quantity given and whether the prescription was collected or delivered. This data was then collated and analysed. Results: Between June 2022 and May 2023 specialist pharmacists prescribed over 1104 emergency prescriptions, on average 5 prescriptions per working day. The main reason for prescription requests were that follow up appointments were booked >6 months ahead (maximum Rx duration is 6months), although travel, lost medication and failing to collect dispensed medication were also common. Conclusions: The 1104 prescriptions written by pharmacists, saved 550 hours of emergency clinic time. Prescriptions, in this clinic, are written for a 6-month duration with people’s follow up appointment booked before the prescription is completed. This data confirms many people have been unable to book their appointments in line with their prescription likely due to operational issues and clinician availability. As first port of call specialist pharmacists allow people to obtain prescriptions on an emergency basis, avoiding ARV interruption and giving an opportunity to counsel patients on adherence, through brief interventions.
