Thus, in case of an already existing or newly diagnosed HIV infection, they rely on voluntary and free support from the community or allied physician. Checkpoint BLN, in cooperation with the Clearingstelle Berlin, has launched an access programme for HIV-infected people without health insurance in 2020, which offers free access to health care. Method: We conduct a descriptive analysis of patient records. All patients presented between 01/01/2021 and 31/12/2022 and were seen at least once by a social worker and an experienced HIV physician at Checkpoint BLN. Patients had to have lived in Berlin for six months. Results: We enrolled 118 PLWHA with a mean age of 33 years (IQR: 29, 39). The participants (pt) were predominantly male (83 %). At baseline, 25/118 pt (21%) without legal residence status and 100/118 individuals (85%) without health insurance. 18/118 people (15%) were sex worker and 43/118 people (36%) reported substance use. We screened 85/118 people for STIs). We found 30/85 (35%) with at least one STI: active Sphilis: 16/85 (19%), Gonorrhoea: 16/85 (19%) and Chlamydia Infection: 12/85 (15%). Hepatitis B antigen was positive in 3/42 (7%) and HCV-Ab was positive in 5/80 (6%), with 2/80 (3%) replicating HCV. Conclusions: In this PLWHA cohort, we found a high prevalence of STIs and blood-borne viral co-infections, indicating the urgency of equal access to health care and appropriate medical treatment regardless of health insurance status. Currently, access to health care in Berlin is only through this special programme for PLWHA without health insurance. Due to the high public health interest in the care of PLWHA without health insurance, this programme should be financed by the public health service in order to meet WHO SDGs goals.
