EACS2023: 608 Ensuring the continuity of ART during significant internal migration and disruption of the sustainable functioning of HCFs in the frontline and occupied territories of Ukraine S. Chernenko Public Health Center of the MoH of Ukraine, Kyiv, Ukraine General data Abstract category: Access and delivery of care Abstract body Purpose: As of June 2023, russia has already damaged almost 1,433 HCFs in Ukraine, including completely destroyed 177 hospitals, 450 pharmacies, 90 ambulances, etc. The military aggression caused the humanitarian crisis and significant internal and external migration of the population, including PLWHA on ART, which required urgent programmatic steps to ensure sustainability of services. Method: Descriptive statistics used data on location of ART dispensing HCFs from the national database (IS HIV). To determine the migration of patients, the location of the HCFs was analyzed. Timeframe: February 01, 2022 (pre-war time) - May 01, 2023. The data withdrawal - June 26, 2023. Results: Before the war, 126,477 PLWHA on ART were registered at different HCFs in all regions of Ukraine. Since February 2022 17,686 PLWHA on ART were deregistered, and only 4,977 PLWHA on ART were registered at new HCFs in the country. The largest negative migration is observed from the front-line eastern regions of the country and was mainly carried out to the western regions, the most distant from the zone of military operations, and nearest to the EU. To ensure sustainability of services a number of urgent program measures were taken: simplified registration of IDPs, Constant information, website (https://phc.org.ua), online-platform (https://findart.phc.org.ua) MMD of ARVs for 6-12 months. Online consultations (https://help24.org.ua), telemedicine, Optimization of ART regimens (TLD-transition), transfer to fixed dose combination, Uninterrupted supplies of ARVs, Active searching of LTFU. Conclusions: Despite the unprecedented challenges, thanks to the help of national and international partners (PEPFAR, Global Fund, etc.), the country managed to maintain the stability of medical care for PLWHA at controlled territories. However, the war and constant russian-rocket attacks on civilian objects continue throughout the country, hence there are a lot of challenges remains, including ongoing migration, HCFs restoring, ARVs supply to de-occupied territories, prevention of HIV resistance, mental health issues.
