Method: A sample of 15 countries was selected according to different domains, to exemplify a variety of contexts (see Figure 1). A targeted search of policy targets was conducted to identify policies that directly or indirectly impact HIV treatment and care. Databases were constructed using a standardized coding scheme and data were reviewed by a second researcher for accuracy. A targeted review of peer-reviewed literature was conducted to contextualize findings from the policy scan, as well as relevant aspects of policy implementation. Figure 1. Approach Results: All countries have policies on immediate initiation of HIV treatment regardless of CD4 count, with regular viral load monitoring (see Table 1). In many cases, these policies aim to address not only morbidity and quality of life of people living with HIV, but also to prevent transmission. All countries have policies that facilitate treatment and care including universal coverage for HIV treatment, and most countries guarantee access to HIV treatment for vulnerable populations, such as migrants or incarcerated people. Despite robust policies regarding HIV treatment and care in many countries, published evidence highlights gaps between official policies and real-world treatment and care provision, in addition to, budget constraints negatively impacting HIV treatment and care. There is variation regarding clinical treatment within countries, especially those with decentralized health systems. Table 1. Findings Conclusions: There are ample policies seeking to facilitate HIV treatment and care in the European region. However, the extent to which these policies exist in theory versus in practice varies significantly between and within countries. Increased budget allocations and uniform clinical treatment could amplify the impact of existing HIV treatment and linkage to HIV care policies, contributing to achieving the 95-95-95 targets.
