Method: We conducted a multicenter, retrospective cohort study including consecutive previously virologically controlled adults PLWH exposed to the same antiretroviral combination for a 2-year period from 2010 to 2020. The primary endpoint was 2-year weight change from baseline. Demographic, clinica, biological and therapeutic data were collected from routine clinical visits. Factors associated with weight change were explored by univariate analysis and by multivariate generalized linear model with covariance analysis. Results: We included 141 ARV regimens in 118 adults. 59% were male, 17% were black ethnicity, mean (SD) age was 51.8 (11.0) the median from diagnosis to ART initiation was 4-y, CDC class was A, B and C in 49%, 27% and 24% respectively. The majority of switches to Doravirine-based ART were from INSTI. Age, gender, black race, baseline CD4 count < 200/µL, nadir CD4 cell count were not significantly associated with 2-year weight while the number of previous antiretroviral lines (PRIORLINES) was (Figure 1, rho, 0.28, p<0.001). The following 2-year exposure to antiretroviral drugs were associated with weight change: Doravirine (n=45), -2.8 kg, 95%CI [-4.9 to -0.8], TDF (n=81), -1.2 kg 95%CI [-2.5 to 0.1], Dolutegravir (n=34), 1.0 kg, 95%CI [-0.5 to 2.4], Rilpivirine (n=21), 1.8 kg, 95%CI [0.4 to 3.2]. Exposure to Doravirine-based antiretroviral combination (Figure 2) was significantly associated with lower weight change (-2.6 kg, p=0.02) than other combinations, after adjusting for PRIORLINES and baseline weight. Conclusions: In our ongoing observational study, doravirine and TDF were associated with a better weight change profile than other antiretroviral combinations. General conditions 1.
