D'Annunzio", Department of Medicine and Science of Aging, Clinic of Infectious Diseases, Chieti, Italy General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: In order to minimize the impact of cART, two-drug (2DRs) strategies have become widespread in recent years, particularly DTG-based regimens. However, the effects of deintensification on the inflammatory context are currently unknown and such a strategy could cause an increase in the inflammatory state. Therefore, we verified the inflammatory status in HIV positive subjects with virological suppression in 2DR based on DTG compared to subjects in classic (3DR). Method: This is an observational study conducted on outpatients in the Infectious-Diseases Clinic, University "Gabriele D'Annunzio" Chieti. We included patients with age >18 years, undergoing cART, without any change in the last 48 weeks, HIV RNA undetectable in last two previous relevations. Results: A total of 116 subjects were enrolled in the study; 89 (76.7%) were male. 82 subjects were taking cART characterized by 3DRs while 34 subjects were taking 2DRs DTG based. The median years since the diagnosis of HIV infection was 15 years (IQR 8.5-24.0). The comparison analysis did not reveal any statistically significant differences in the two study groups for socio-demographic characteristics, specifically for gender, age and IL-1β, IL-10, IL-33, and IFN-γ levels. Significant differences were found between the two groups only in the levels of IL-18 (648.8 in 3DR vs 475.0 in 2Dr group p=0.03) and IL-36 (281.7 in 3DR vs 247.0 in 2DR p=0.05). No correlation between cytokines levels, biochemical and virological parameters was found. Conclusions: This study confirms the absence of significant differences in IL-1β, IL-10, IL-33, and IFN-γ levels between two groups. Furthermore, subjects in 2DRs show greater control over IL-33 and IL-36 serum levels, cytokines related to an increased cardiovascular risk and chronic diseases related to ageing. This demonstrates that regimens featuring two antiretroviral agents DTG based are associated with a better control on chronic inflammation that characterizes PLWHs in effective ART.
