EACS2023: 778 Cardiovascular disease prevalence in a cohort of HIV positive (PLWH) patients, over 50 years of age, in the era of the INSTI based regimen: a proof of concept retrospective analysis M. However, several studies have linked INSTI to an increased incidence of cardiovasclar (CVD) events. Here we examined the risk of CVD in a population of PLWH over 50 years of age, living in a highly industrialized area with higher prevalence of CVD events in the general population. Method: In this retrospective, monocentric, proof of concept analysis, we randomly selected 100 PLWH from the 2500 patients enrolled within the MASTER cohort, followed in our Unit of Infectious and Tropical Diseases. Inclusion criteria were regular follow-up and age >50. Primary endpoint consisted in CVD event. Logistic regression was used to analyze variables related with CVD event. Analysis was adjusted for: anthropometric, socio-demographic, risk factors, clinical characteristics, immunovirologic data and treatment history (INSTI and PI). Results: In our cohort (77% males, 23% females, mean age 59 y), 14 patients (3 females, 11 males, OR 0,8CI 0,2-3,4) experienced a CVD event (12 myocardial infarction, 1 pulmonary embolism, 1 stroke); 36% had hypertension, 7% diabetes and 5% CKD; 8 patients had BMI>30. Median CD4/CD8 nadir: 0.3 (IQR 0.1-0.4), median CD4% nadir: 14.8% (IQR9.2-21.7), viral load zenith was 99.000 cp/ml (IQR 32000-279524). 4 patients had AIDS defining events; 91 patients received INSTI (23 bictegravir, 60 dolutegravir, 8 raltegravir), 88 had history of PI exposure; 10/14 patients were taking INSTI during the CVD event. Age [AOR 4,2(IC95%1,2-14,0)] and diabetes [AOR 6,0(IC95% 1,0-34,3) p0,004] appeared to be the only variables with independently related correlation. Conclusions: In this preliminary analysis the number of CVD events appears very high. Age and diabetes are the only variables with independently related correlation. Previous exposure to INSTI was frequent but does not seem correlated with a higher risk. More in-depth analysis is ongoing on a larger cohort. General conditions 1.
