Method: A retrospective monocentric cohort including virologically-suppressed (HIV-RNA<50 cp/mL) patients switching to TAF/FTC/BIC in 2018-2022 was performed. Probabilities of virological failure (VF, i.e. 2 consecutive HIV-RNA ≥ 50 cp/mL or single HIV-RNA ≥ 200 cp/mL) and of treatment discontinuation (TD) were estimated by Kaplan-Meier. Predictors of both outcomes were identified through Cox regression. Analysis-of-variance for repeated measures was used to examine changes in CD4-to-CD8 ratio from baseline up to 36 months and predictors of its variation were analyzed through linear regression. Results: Overall, 16 VF occurred during 22 months of median follow-up. Estimated probabilities of VF at 1, 2 and 3 years were 2.0% (95% CI 1.0-4.2%), 2.9% (95% CI 1.5-5.6%), 5.5% (95% CI 3.2-9.2%). Caucasian ethnicity (versus others, aHR 0.32, 95% CI 0.11-0.94; p= 0.039) and previous VF (versus none, aHR 10.06, 95% CI 1.32-76.56; p = 0.026) independently predicted VF. Particularly, switch from any prior drug class didn't predict VF. Overall, 42 TD occurred: predominantly for simplification, followed by toxicity and one for VF. No predictors of TD were identified; the safety was similar across different pre-switch strategies. An increase in CD4-to-CD8 ratio over 3 years was evidenced (p<0.001). Previous AIDS-event (versus none, B: -0.09, 95% CI -0.17; 0.00; p=0.047) and longer time of virological suppression (per 1 year more, B: -0.02, 95% CI - 0.03; -0.01; p<0.001) were associated with a reduction in delta CD4-to-CD8 at 12 months. Total cholesterol decreased over 3 years (p<0.001). Higher zenith HIV-RNA predicted a lower decrease (>500,000cp/mL versus<100,000cp/mL, B:+16.9; 95% CI +5.4; +28.4; p=0.004), whereas switching from a 2-drug regimen (versus a 3-drug regimen, B: -15.3; 95% CI -29.4; -1.13; p=0.034) was associated with greater decrease. Triglycerides didn't change over time (p=0.465). Patients’ characteristics are in table 1. Conclusions: BIC/FTC/TAF demonstrated effectiveness, tolerability and safety. General conditions 1.
