EACS2023: 565 Inclusion of women, Black and People of Colour (BPoC) and older adults in real-world and implementation studies evaluating longacting injectable (LA-I) Cabotegravir (CAB) and Rilpivirine (RPV): a rapid review A. Orkin Queen Mary University of London, Blizard Institute, Department of Infection and Immunity, London, United Kingdom, SHARE Collaborative, London, United Kingdom, Barts Health NHS Trust, London, United Kingdom, Queen Mary University of London, Wolfson Institute of Population Health, London, United Kingdom General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: Globally, 54% of people living with HIV are female; 2/3 are from the WHO African region, and a growing proportion are older adults (UNAIDS, WHO). However, inclusion in phase III LA-I CAB/RPV trials (FLAIR, ATLAS, ATLAS-2M, SOLAR) did not reflect this: Female 25%, Black and People of Colour (BPoC) 28%, adults > 50 years 18%. Method: We conducted a rapid review to evaluate proportions of women (cis and transgender), BPoC and older adults included in LA-I CAB/RPV implementation studies and real-world studies/series (IRWS). We searched electronic databases (MEDLINE, Embase) and conference abstracts for peer-reviewed manuscripts/presentations. We report on participant demographics presented in baseline characteristic tables/text. We describe female inclusion with respect to proportions of females living with HIV (FLWH) in that country/region. Results: We included 14 studies with sites in 10 high-income countries [North America (n=110), Europe (n=45, and East Asia (n=1)]. Of the studies, 5 were described as ‘implementation’, 9 were ‘real-world’; 8 were single-site, 6 multi-site. 1913 people received LA-I. All 14 studies reported on participant sex. 9 studies failed to report whether/not transgender women (TGW) were included, 3 did not report race and ethnicity, 10 did not report on proportion of adults over 50 yrs [Table 1]. Of the 1913 participants, 17% (324/1913) were female (range 6-43%), 1.8% TGW (16/902) (range 1-4%), BPoC 47% (738/1583) (range 22-91%); The weighted median age was 42 (n=11). 11/14 studies included lower proportions of female participants than the proportion of FLWH in their country/region [Table 2]. Conclusions: Currently, participants receiving LA-I CAB/RPV in IRWS are not reflective of global or country-level populations of FLWH. Several studies failed to describe inclusion of important demographic groups in baseline demographic tables/text. We recommend future studies report on all characteristics and better represent both the population of people living with HIV locally and globally. General conditions 1.
