Xiong The First Affiliated Hospital, Zhejiang University School of Medicine, The Department of Infectious Diseases, Hangzhou, China General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: This is a prospective study aiming to characterize and compare the plasma lipidomic profile of HIV patients taking EVG/c/FTC/TAF or BIC/FTC/TAF in a real-world clinic population. Method: Patients ≥ 18 years old on EVG/c/FTC/TAF regimen with HIV-RNA < 50 copies/mL for ≥6 months were recruited into the study and were followed every 12 weeks (N=48). Patients were divided into two groups depending on whether switching to BIC/FTC/TAF regimen during follow-up. Lipidomic analyses conducted by liquid chromatography–mass spectrometry (LC-MS) were performed at baseline and after 12 and 24 weeks. Results: No significant differences (97.7% male, mean age 34 years old, 16.6 months receiving EVG/c/FTC/TAF, CD4+ T cell 560/ul, TG 1.64, TC 4.92, LDL-C 2.93, HDL-C 1.19 mmol/L) were observed between EVG/c/FTC/TAF (N=25) and BIC/FTC/TAF (N=19) group at baseline. At 24 week, patients who switch to BIC/FTC/TAF had lower TG (1.23 VS 2.03 mmol/L P=0.001) and higher total bilirubin (14.0 vs 10.4 umol/L P=0.039). Of note however, significant reduction in TC and LDL-C was observed in both groups. Additionally, a total of 1158 plasma lipid molecular species were identified and quantified. In both treatment groups, a large number of lipid molecular species were altered including glycerolipids, glycerophospholipids, sphingolipids during the 24-week follow-up period, nevertheless, EVG/c/FTC/TAF had a more significant impact on lipid molecules species compared with BIC/FTC/TAF therapy, especially in phospholipids and sterol lipids. EVG/c/FTC/TAF treatment markedly increased plasma level of lyso-phosphatidylcholines (LPCs), diglycerides (DGs), triglycerides (TGs). DG(36:2), DG(36:3), DG(36:4), LPC(22:6)+CH3COO and LPE(22:6) was found to be positively correlated with 10-year cardiovascular risk. Conclusions: Despite minor changes in routine clinical lipid parameters, EVG/c/FTC/TAF treatment had significantly elevated lipid species associated with cardiovascular risk.
