Montes Hospital Universitario La Paz, IdiPAZ, Internal Medicine Department / HIV Unit, Madrid, Spain General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: Long-acting cabotegravir a​​​​​ nd rilpivirine (LA-C&R) have demonstrated efficacy, safety and improved quality of life of people living with HIV. However, there is a lack of data on effectiveness and follow-up in a real-world setting. This study aims to monitor the effectiveness of LA therapy at 3 and 6 months after switching from standard oral therapy and determine the optimal timing of viral load (VL) measurement after 2 or 3 injection after switch. Method: We conducted a retrospective observational unicenter study, including LA-C&R initiations between January to May 2023, excluding subjects from previous clinical trials. Viral load measurements were performed around the 2 or 3 LA administration, based on clinician discretion. Clinical and microbiological data was collected from medical records. Descriptive analysis of factors associated with VL ≥50 copies/mL was performed. Detailed comparison of viral loads will be provided in the final version. Results: Out of 139 subjects included in the study VL measurements were available in 61 participants at the time of analysis. Five participants had NNRTI resistance mutations. (Table 1). Following LA-C&R switch, 95% of subjects had undetectable VL (<50 copies/mL). All participants with detectable VL (≥50 copies/mL) had experienced blips two years prior to the analysis and the proportion of blips was similar regardless of the timing of VL measurement (<45 or >45 days after LA initiation). Within this subgroup, three subjects had detectable VL in the 1 month, but remained undetectable during follow-up. Four subjects had detectable VL prior to switch; three remained undetectable after switching. (Table 2). Conclusions: LA-C&R therapy appeared effective in the short-term even in PLWH with exclusion criteria from clinical trials, with cautious considerations for individuals with previous blips.
