Cassetti Helios Salud, Buenos Aires, Argentina General data Abstract category: Antiretroviral therapy – observational studies Abstract body Purpose: To describe the impact of bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF) fixed-dose combination (FDC) on metabolic profile and renal function in a cohort of treatment-experienced (TE, switching patients) and treatment-naive (TN) people living with HIV (PLWH) in Argentina (BICTARG study). Method: Observational retrospective cohort study in a reference HIV clinic in Buenos Aires, Argentina (period 10/2019 to 12/2021). TE (n=2356) and TN (n= 315) PLWH receiving B/F/TAF FDC were included. We compared fasting glucose, total cholesterol (TC), HDL-cholesterol (HDL-c), and LDL-cholesterol (LDL-c) and estimated glomerular filtration rate (eGFR) at baseline vs. 24 and 48-week follow-up. Results: In the TE cohort, 38.2%, 34.3%, and 7.8% switched from boosted-PI, NNRTI, and INSTI-based regimens, respectively; 63.9% had comorbidities. Baseline laboratory parameters were available for 1542 PLWH. No statistical differences were observed at 24 and 48 weeks in median glucose, TC, HDL-c, and LDL-c, while triglycerides decreased (p<0.001), as shown in Table 1. In TN, mild increases in median values from baseline were observed for TC, HDL-c, and LDL-c at 24 weeks (158 vs. 175; 38 vs. 42; 95 vs. 106 mg/dL, respectively; p<0.05) and in TC and HDL-c at 48 weeks (158 vs. 169.5; 38 vs. 41 mg/dL, respectively, p<0.05), without changes in glucose or triglycerides. In both populations, a mild decrease in eGFR at 24 and 48 weeks was documented (p<0.001). Conclusions: In this large Latin-American cohort of TE PLWH, switching to B/F/TAF FDC had a neutral impact on cholesterol values and a reduction in triglycerides. In the TN cohort, we observed a mild increase in cholesterol values but without variations in glucose or triglycerides. The eGFR decrease was consistent with the expected inhibition of the tubular secretion of creatinine by bictegravir (without affecting renal glomerular function). This real-world data supports the safety and favorable metabolic profile of this FDC in a Latin American cohort. General conditions 1.
