Our aim was to monitor the blood-associated total HIVDNA level over 28 weeks in a setting of ART-experienced complex PLWH followed by our home-care assistance, who switched from standard ART to CAB+RPV long acting injectable (LAI). Method: This was a prospective, longitudinal study. None of the participants underwent the lead-in phase. We quantified total HIV-DNA in the blood by digital droplet PCR and monitored the viro-immunological profile at different time points, from baseline (time of switch, BL) to 28 weeks of follow-up. Total HIV-DNA levels were expressed as log copies/10 leukocytes. Results: We enrolled ten participants: mostly males (60%), Caucasians (80%), viro-suppressed (HIV-RNA <50 cps/mL) (70%), with a median age of 58 (IQR 50-72) years. The participants’ characteristics at BL are summarized in Table 1. Median BL HIV-DNA levels were 2.31 (IQR 1.93-2.50) and an increase up to 2.39 (IQR 1.93-2.69) occurred at 2W. However, HIV-DNA levels returned to lower values at 4W, 2.19 (IQR 2.00-2.56), remaining stable around the median baseline levels until 28W, 2.28 (IQR 1.87-2.54) (Figure 1). Regarding to the viro-immunological status, CD4 cell count increased from a median value of 422 (IQR 220-676) at BL to 509 (IQR 212-699) at 28W, while both virological profile and CD4/CD8 ratio remained stable over the study period. Conclusions: Overall, we observed that this novel strategy with CAB+RPV LAI combined with home-care assistance seemed to have an overall control on both the levels of the cellular reservoir and the viro-immunological profile in this complex setting of PLWH. However, these exploratory data need to be confirmed in a larger cohort and with a longer follow-up. General conditions 1.
