The high potency and efficacy rates of bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF) in clinical trials make it a promising strategy for this group of patients. Real-life data regarding this aspect is scarce. Method: We performed a multicenter, non-controlled, retrospective study of pretreated and virologically non-suppressed PLWH switching to B/F/TAF. We evaluated the percentage of virological suppression at weeks 24, 48, and 96 after switching and the immunological improvement. Results: The study comprised 123 persons from 8 hospitals in Spain, of whom 80.5% were men and had a median age of 52.0 [42.0 – 57.0] years. The median time of HIV infection was 30.0 [24.0–39.0] years, and previous AIDS diagnoses had been made in 25.6% of cases. Virological control of HIV-1 was achieved in 74.7% (65/87) of cases at week 24, 72.9% (86/118) at week 48, and 88.6% (101/114) at week 96. Median CD4 at switching to B/F/TAF was 675 [386 – 1000]. In week 48, we found a decrease in CD8+ count of -63 cells/mm (-316 – 101) and an increase in CD4+ count of 39 cells/mm (-98 – 166) compared to baseline. In week 96, we found a decrease in CD8+ count of -9 cells/mm (-316 – 101) and an increase in CD4+ count of 37 cells/mm (-132 – 214) compared to baseline. CD4/CD8 ratio increased by 0.08 (-0.03 – 0.24) and 0.06 (-0.12 – 0.21) in weeks 48 and 96, respectively. In AIDS-diagnosed patients, an increase in the CD4+ count of 76 (-78 – 167) cells/mm was found at week 48. Conclusions: In PLWH with persistent low-level viremia, B/F/TAF achieved high rates of virological control and slightly improved the immune status in terms of CD4+ during weeks 48-96 after switching.
